Causal Factors

This report provides a list of causal factors with the number of times each causal factor was reported in an event. The system uses causal factors that are recorded in events because safety supervisors have verified them and/or identified them through investigation. Causal factors selected only on the supervisor report are not reflected on this report.

Note: This functionality is available only to customers who have licensed the Incident Management System (IMS) module in PureSafety.

The report includes three views: Summary Level, Primary Level, and Detail Level. In the example below, each view provides varying detail for the causal factor, Employee.

Report Parameters

Parameter	Default Value	Details
Location	None selected	All locations are included by default.
Period	Month	Required. This selection changes the next two options.
Month	Jan	Only displayed if Month is selected in the Period parameter field.
Quarter	Quarter 1	Only displayed if Quarter is selected in the Period parameter field.
Year	[Current Year]	Only displayed if Year is selected in the Period parameter field.
Printing Options	Standard Page Break	You can select a standard page break or a page break after each location.
Level of Detail	Summary Level	The Summary Level view includes a complete list of categories and causal factors. The Primary Level view includes a complete list of categories. The Detail Level view includes the categories with their associated causal factors beneath ordered by percentage.
